SCP Drive through Incidents

Do you have a procedure for investigating incidents on site? Presently we report more serious incidents to the Police but have been told we also need to follow an internal investigation, something we have never done. Any guidance much appreciated.

Hi Corinne,

We (in Hackney) are expected to complete a report on our intranet for any incident and submit this to the health and safety team. Once this has been done they ask us to complete an investigation following their pro forma (which I have not attached but might be able to send later if needed).

Much the same as Hackney, I fill out a simple report form to our H&S team for any near misses, and keep a copy for myself so I can see if there is a pattern at a particular site, in which case I would take action. In most cases they are random events, at different sites, but isolated. More serious incidents are more fully investigated, and if we have driver details, reported direct to the Police. I think as long as you have a procedure that logs all incidents, so that you can take action if necessary, that should suffice. It is very difficult to instigate an internal investigation for every random drive through in my view, unless you have comprehensive details and witnesses.

Hi Corinne,

Similar to above, we have an intranet 'online reporting tool' for all H&S incidents. I also keep my own notes so I can examine the data for each patrol and each site if required.

In addition, we ask patrols to report all drive through incidents to the local safer roads partnership, if they or a witness have managed to jot down the driver/vehicle details. This itself can be problematic, as I've had an situation where the patrol and 2 independent witnesses provided details for a serious incident, but as the 3 individuals saw 3 different vehicle makes and models (and even colours!), a driver who was both male and female, the police were not able to take the matter further.

SCP Drive through Incidents

Hi Corinne,

All drive through incidents are reported to Sussex Police for them to investigate. I complete a form and it is sent to the traffic section. I also can report incidens on Operation Crackdown which has a section for SCP incidents.

I report it to Health & Safety if the incident involves injury to staff or children although that is being reviewed at the moment. My line manager would then investigate the incident. I keep records of all incidents and outcomes.
